Perhaps an early military example of a ‘desire line’ (design that recognises how users behave, rather than how you would have them behave).

British soldiers in tanks would park and exit the tank to ‘brew’ tea, leaving them dangerously exposed.

Solution? Install a hot (boiling) water system inside tanks.
